Kirby Medical Center Awarded PCMH Exemplary Provider® Status

SPRING HOUSE, PA (April 2017) – Kirby Medical Center of Monticello, Illinois, was recently awarded Exemplary Provider® accreditation status for Patient Centered Medical Home (PCMH) by The Compliance Team, Inc.

PCMH Exemplary Provider accreditation is only granted to providers that demonstrate 100% compliance with Improving care delivery practices while responding to each patient’s unique needs and preferences. The PCMH program includes comprehensive, patient-centered, coordinated care which includes enhanced accessibility, quality and safety. The Compliance Team’s proprietary operations-driven, nationally recognized accreditation is a comprehensive, continuous quality improvement program that standardizes the provider’s policies and procedures, streamlines its operation and validates its quality. This PCMH accreditation model is built around enhanced communication with their patients and focuses on what matters most to patients and their families.
