Memorial Medical Clinics Awarded PCMH Exemplary Provider® Accredited Status
SPRING HOUSE, PA (June 2017) – Memorial Medical Clinics in Bowen, Nauvoo, Carthage, Colchester and LaHarpe, Illinois were recently awarded Exemplary Provider® accredited status for demonstrating outstanding compliance to comprehensive Patient-Centered Medical Home (PCMH) accreditation requirements. The clinics are part of Memorial Hospital of Carthage, Illinois.
The PCMH program includes comprehensive, patient-centered, coordinated care which includes enhanced accessibility, quality and safety. Memorial’s PCMH designations mark its second set of accreditations through The Compliance Team, Inc. The clinics were previously awarded The Compliance Team’s Medicare-approved Rural Health Clinic (RHC) accreditations and are now federally designated Rural Health Clinics.
The Compliance Team’s PCMH accreditation validates quality by having healthcare delivery organizations set-in-place an operations-based program that focuses on process simplification and standardization. Special emphasis is given to enhancing patient communication and follow-up as well as coordination of care within a patient’s healthcare provider community who work together on meeting that patient’s unique needs and preferences.
